KHVVG / KHAG · Funding in Germany
Plan the digital future of surgical care within the Transformation Fund
From 2026 to 2035, the German federal government and the federal states are providing up to €50 billion to restructure the hospital landscape. medudoc moves preparation for the procedure to the patient's home – with the same standard at every site.
Transformation Fund
What the Transformation Fund provides
The fund supports projects by hospitals in Germany that change care structures. It replaces the Hospital Structure Fund (Krankenhausstrukturfonds) and runs for ten years.
- funding volume for restructuring the hospital landscape, provided by the federal government and the federal states
- €50 billion
- term of the fund
- 2026–2035
- federal share from the special fund for infrastructure and climate neutrality
- €29 billion
- Hospital operators submit projects to their federal state. The state applies to the Federal Office for Social Security (BAS).
- State → BAS
- From 2026 to 2029, the federal states, where applicable together with hospital operators, bear at least 30% of the eligible costs, and from 2030 at least 50%.
- The BAS reviews the applications, issues the funding decisions and approved the first funds on April 17, 2026.

Legal framework
KHVVG and KHAG: how the two laws are connected
The German Hospital Care Improvement Act (KHVVG) launched the hospital reform and created the Transformation Fund. The German Hospital Reform Adjustment Act (KHAG) adjusts this reform without replacing it. Both laws apply together to your project.
- KHVVG
Krankenhausversorgungsverbesserungsgesetz (German Hospital Care Improvement Act)
Sets out the foundations of the reform.
- Divides hospital services into service groups with quality requirements.
- Introduces standby funding (Vorhaltevergütung), which pays part of the budget independently of case numbers.
- Creates cross-sector care facilities.
- Establishes the Transformation Fund with eight funding categories (Section 12b of the Hospital Financing Act, KHG).
- KHAG
Krankenhausreformanpassungsgesetz (German Hospital Reform Adjustment Act)
Adapts the reform to implementation in the federal states.
- Revises service groups and quality criteria.
- Introduces standby funding at a later date.
- Extends exemptions and cooperation options for the federal states.
- Finances the federal share of the fund from the special fund instead of from statutory health insurance funds.
The funding categories come from the KHVVG. Financing and deadlines follow the version as amended by the KHAG.
Funding categories
Where medudoc fits into the Transformation Fund
The fund has eight funding categories (FTB). These five are particularly relevant to preparation for surgery.
Cross-site concentration of acute inpatient care
When services are concentrated at fewer sites, journeys become longer. With medudoc, patients complete the medical history and patient education video from home; the consent discussion is also possible telemedically. This saves journeys before the procedure.
Conversion into cross-sector care facilities
Where outpatient and inpatient care grow together, preparation must be completed before admission. medudoc brings together medical history, findings and consent before patients enter the hospital.
Telemedical network structures
Telemedicine makes expertise independent of location. With medudoc, a central anaesthesia team reviews complete cases from all sites and resolves queries in a targeted way.
Regionally limited hospital networks
In a network, shared standards count. medudoc maps a uniform preparation pathway for each procedure, which every hospital uses with its own branding and its own contacts.
Centres for rare, complex or serious diseases
Patients often travel long distances. Complete, multilingual preparation in advance relieves the centre and the day of admission.
Areas of application
Putting it into practice: three areas of application for your project
Uniform surgery preparation across the network
One standard per procedure – the same at every site.
What you implement
- Shared templates for medical history, patient education and consent
- Central specifications, adaptable locally
- Uniform evidence through audit trail and consent trail
Your benefit
- The same quality of preparation in all hospitals
- Fewer journeys despite longer distances
- Comparable workflows across the entire network
Connecting outpatient and inpatient care
Preparation completed before patients arrive.
What you implement
- Medical history, findings and consent before admission
- Upload findings from practice and laboratory by photo
- Preparation plan with reminders up to the day of surgery
Your benefit
- Less information lost at the boundary between sectors
- Predictable workflows for outpatient procedures
- Fewer appointments before the procedure
Sharing anaesthesia expertise
One team reviews cases from all sites.
What you implement
- Review and clear complete cases centrally
- Answer questions from the patient education video in a targeted way
- Follow-up phone call directly from the case
Your benefit
- Specialist review for smaller sites too
- Well-prepared consent discussions
- Relief through complete documents
Frequently asked questions
Questions about the KHVVG, KHAG and Transformation Fund
What is the difference between the KHVVG and the KHAG?
The German Hospital Care Improvement Act (Krankenhausversorgungsverbesserungsgesetz, KHVVG) has been in force since the end of 2024 and forms the basis of the hospital reform – with service groups, standby funding and the Transformation Fund. The German Hospital Reform Adjustment Act (Krankenhausreformanpassungsgesetz, KHAG) has applied since April 15, 2026. It amends the KHVVG in specific areas, such as deadlines, exemptions and the financing of the fund.
Who applies for funds from the Transformation Fund?
Hospital operators submit their project to the responsible ministry of their federal state. The state selects projects and applies for the funds to the Federal Office for Social Security (BAS). The BAS reviews and approves; the state passes the funds on to the hospital operator.
Which funding categories are there?
Section 12b of the Hospital Financing Act (KHG) lists eight funding categories: cross-site concentration, cross-sector care facilities, telemedical network structures, centres for rare, complex or serious diseases, regional hospital networks, integrated emergency care structures, the closure of sites and additional training capacity.
Is digitalisation funded on its own?
No. Unlike under the Hospital Future Act (Krankenhauszukunftsgesetz, KHZG), digitalisation is not a funding purpose in its own right. It counts insofar as it supports a project that changes care structures, for example a network or a cross-sector facility.
How high is the federal states' own contribution?
The federal states, where applicable together with the hospital operators, contribute to every project – from 2026 to 2029 with at least 30% of the eligible costs, from 2030 to 2035 with at least 50%.
Is medudoc eligible for funding under the KHAG?
A short legal opinion by the law firm Lyck+Pätzold. healthcare.recht concludes that medudoc can be eligible for funding under certain conditions, particularly in funding category 3. What matters is its integration into a specific project. The federal state and the BAS decide on eligibility for funding on a case-by-case basis. On the assessment in the legal opinion
