A patient education video improves comprehension before total hip replacement

In a randomised controlled trial by the Department of Orthopaedics at the University of Würzburg, patients received a patient education video in addition to standard patient education before total hip replacement. Afterwards, they answered significantly more knowledge questions correctly – self-assessment and emotional well-being remained unchanged.

Standard patient education with and without video

All patients received regular patient education on the day of admission with a standardised consent sheet and a discussion with a physician. The intervention group additionally watched a patient education video in advance. Allocation was random.

More correct answers with video

Patients with video answered 86.5% of the knowledge questions correctly, without video 78.6% (p = 0.014). The differences were significant for treatment alternatives (p = 0.009) and the risks of the procedure (p = 0.016) – content that is particularly important for valid consent.

Knowledge increases, not just the feeling of being informed

Both groups rated how well informed they felt almost identically (p = 0.986). Anxiety, satisfaction with patient education and preparation for the procedure did not differ either (p = 0.333). The video therefore improved actual comprehension – without causing additional uncertainty.

Einordnung

What the study shows – and what it does not

Das zeigt die Studie

  • An additional patient education video significantly improved objective comprehension before total hip replacement.
  • The differences were most evident for treatment alternatives and risks.
  • Self-assessment and emotional well-being did not change as a result of the video.

Grenzen der Studie

  • Small evaluated sample – 37 of 74 enrolled patients with a complete knowledge questionnaire.
  • Single-centre, one procedure; the questionnaires were developed by the authors.
  • Measured directly after patient education; long-term knowledge and clinical outcomes were not examined.
  • The video supplemented the standard process; whether it shortens discussion time was not part of the study.
